Thank you for partnering with Memorial University Medical Center (MUMC). In order to maintain a safe, secure environment for our patients, visitors, Team Members, and vendors, we have specific guidelines regarding access and movement throughout MUMC. When conducting business within our facility, it is important that all representatives of your business understand and abide by our vendor policies and procedures. All of the information can be downloaded below.
We created our Vendor Access Management Program to provide a higher level of customer service to all of our vendors. This program replaces all former vendor management systems at MUMC (i.e. Reptrax). With this new program, vendors pay an annual fee of $75.
Vendors participating in operating room or catheterization lab procedures must show proof that they have received a hepatitis B vaccine and a tuberculosis skin test.
Please submit the following information on company letterhead:
- Statement about the representative's qualifications to sell company products
- Proof that the product is FDA-approved
- Proof of training in sterile technique, if applicable
- Proof that representatives are covered by their company's liability insurance
The primary vendor representative must sign a confidentiality agreement and letter of
understanding indicating he/she read, understands, and agrees to abide by the Vendor Access Management Program guidelines. The original signed copies must be returned with payment. Return information in person at the materials management vendor check-in area at MUMC or mail it to:
