What is a typical day like during Memorial
University Medical Center inpatient service?
8 a.m. to 9 a.m., morning report (Mondays, Tuesdays, and Thursdays)
or Grand Rounds (Fridays)
9 a.m. to 12:30 p.m., Internal Medicine Outpatient Clinic (Monday
through Friday), or
9 a.m. to 10:30 a.m., work rounds and inpatient duties and
10:30 a.m. to 12:30 p.m., attending rounds
12:30 p.m. to 1:30 p.m., lunch conference
1:30 p.m. to 5 p.m., Internal Medicine Outpatient Clinic (Monday
through Thursday), or
1:30 p.m. to 3 p.m., work rounds and inpatient duties and
3 p.m. to 5 p.m., attending rounds
What is the On-Call System?
- Inpatient Teaching Services
There are five general ward services, four are internal medicine (yellow, red, blue, and green) and comprised of an attending, resident, two interns, one to two medical students, and occasionally a pharmacy or physician assistant student. The other general ward service is from the family medicine residency. The ward teams take call every fourth night from 7 a.m. to 7 a.m. the next day. The interns are assigned either "first call" or "second call" designation. The second call intern admits patients from 7 a.m. to 7 p.m. The first call intern usually admits patients from 7 p.m. to 7 a.m. and does the general cross-cover. We fully comply with ACGME requirements for residency training programs with particular attention to the number of weekly hours and the number of admissions each intern/resident can have. After a team caps, any further admissions are done by the night float resident. All members of the house staff are given one day off per week averaged over four weeks.
- Night Float
The Department of Internal Medicine has developed a resident night
float system. Sunday through Thursday nights from 7 p.m. to 7 a.m.
is covered by an upper-level resident assigned to the night float
service. Night float takes the place of the on-call resident, allowing
him/her to go home at 7 p.m. Friday and Saturday nights are shared
by upper-level residents assigned to no-call rotations. This system
is strongly supported by the house staff.
- PGY-1
The PGY-1 residents take call while assigned to internal medicine wards. Call is every fifth night, in house, and runs from 7 a.m. to 7 a.m. PGY-1 residents assigned to the critical care rotation take call every fourth night.
- PGY-2 and PGY-3
The PGY-2 and PGY-3 residents take overnight in-house call while on the night float rotation from Sunday through Thursday, 7 p.m. to 7 a.m. PGY-2 and PGY-3 residents assigned to the critical care rotation take call every fourth night.
- Subspecialty/Elective Rotations
Residents on subspecialty services do
not take in-house call, except on the critical care rotation. Generally, the resident will take call with
the respective subspecialist and pick up any new patients admitted
overnight in the morning.
When do you have your continuity
clinic?
Interns and residents spend a half day
each week in their own continuity clinic and provide hospital follow-up
care to patients seen at Memorial University Medical Center. Clinic occurs when the resident
is neither on-call, pre-call, nor post-call. There are three residents
to every one attending in clinic.
