The state of Georgia requires residents to have a residency training permit. To apply for a permit, go to www.medicalboard.georgia.gov.

When completing the residency training permit application:

  1. Page 1 of application:

  2. Page 2 of application:
    If you answer yes to any of the questions on this page, you must provide an explanation in a separate document to be attached to the application.

  3. Form A
    You must provide an actual photo. A paper copy of a photo will not be accepted.
    You must have your signature notarized. Your medical school or local bank should have a notary on staff.

  4. Form B
    Part 1-Complete this section.
    Part 2-Do not complete. Our GME office will obtain the program director’s affidavit and the notary signature.

    Our GME office will mail applications, supporting documentation, and the fee to the Georgia Composite State Board of Medical Examiners. Training permits are mailed to the GME office and then distributed to residents.
