3. Enter requestor name and facility. Full address is optional.
4. Verifying letter will appear on screen verifying any facility affiliations currently in the database. Print the letter.
5. The fact that a doctor is not listed cannot be presumed to indicate there are any problems or concerns since sometimes errors are inadvertently made. If you are inquiring about a doctor whose name is not listed, please contact the Memorial Health Credentials Verification Service directly at (912)350-6664.
