Our business office team is highly trained in customer service and all billing aspects of the healthcare industry. At your first visit, you will be asked to fill out forms and present your insurance card so we can make a copy for our files. It is your responsibility to provide complete and accurate information. Please bring your most current insurance card with you. Our office will file insurance claims for your care. If your insurance changes and a new or updated insurance card is sent to you, please notify our office and bring your updated card to your next visit.

It is important that you know your insurance company's laboratory of choice. Please call your insurance company to verify where lab tests should be sent so that your lab charges are properly covered.
